Roles & Responsibilities

Deliver compassionate direct patient care following facility protocols
Administer medications and treatments safely and accurately
Monitor patient condition and report changes promptly to the care team
Maintain clear, timely documentation in the patient record
Assist with patient hygiene, mobility, and basic comfort needs
Collaborate with nurses and interdisciplinary staff to support patient goals
Support admissions, discharges, and effective patient transfers

Qualifications

Current LPN license in Pennsylvania or compact/licensure acceptable to the facility
Completion of an accredited LPN program
Basic Life Support (BLS) or CPR certification as required by facility
Reliable clinical skills and ability to follow physician and nurse directives
Strong communication skills and a team-first attitude
Previous LPN experience in a clinical or long-term care setting preferred

Education

Graduate of an accredited LPN/LVN program
High school diploma or GED

Knowledge & Skills

Medication administration and documentation best practices
Vital signs monitoring and basic patient assessment
Comfortable using electronic health records and charting systems
Infection control procedures and safe patient handling
Time management and prioritization in a busy clinical environment
Customer-service oriented approach to patient and family interactions
