Roles & Responsibilities
- Provide compassionate direct patient care, focusing on safety and dignity
- Administer medications and treatments accurately according to orders
- Monitor patient status and report changes promptly to the care team
- Document care and observations clearly in the patient record
- Collaborate with nurses and interdisciplinary staff to support care plans
- Assist with admissions, transfers, and discharges to ensure smooth transitions
- Support patients and families with education and a calming bedside manner
Qualifications
- Valid LPN license in Pennsylvania (required)
- Completion of an accredited practical nursing program
- Current BLS/CPR certification (or willingness to obtain)
- Comfortable participating in per diem scheduling and variable shift coverage
- Strong clinical judgment, attention to detail, and a patient-centered attitude
- Previous LPN experience in a long-term care or skilled nursing setting preferred
Education
- Graduate of an accredited Practical Nursing program
- High school diploma or GED required
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Proficient in vital signs assessment and patient monitoring
- Familiarity with documentation best practices and electronic charting
- Excellent communication and teamwork skills with patients and staff
- Ability to prioritize tasks and work efficiently in a fast-paced setting
- Committed to infection control standards and safe patient handling
