Roles & Responsibilities

Provide compassionate direct patient care, promoting comfort and dignity
Administer medications and treatments safely and according to policy
Monitor patient condition and report changes promptly to the care team
Document patient care accurately in the medical record
Collaborate with nurses, therapists, and physicians for coordinated care
Assist with admissions, discharges, and seamless transitions of care
Support patients and families with education and empathetic communication

Qualifications

Active LPN (Licensed Practical Nurse) license in Pennsylvania or eligibility for state licensure
Current BLS/CPR certification (required)
Previous LPN experience in skilled nursing, long-term care, or similar setting preferred
Strong clinical assessment and medication administration skills
Reliable, professional, and patient-centered approach to care

Education

Graduate of an accredited LPN or vocational nursing program
High school diploma or GED required

Knowledge & Skills

Solid knowledge of basic nursing principles and safety practices
Medication administration and infection control procedures
Comfortable with wound care, ADL assistance, and vital signs monitoring
Clear, compassionate communication with patients, families, and team members
Accurate documentation and familiarity with electronic health records
