Roles & Responsibilities

• Provide compassionate direct patient care, supporting daily living activities and comfort
• Administer medications and treatments accurately and safely according to plan
• Document patient assessments, interventions, and responses clearly and promptly
• Collaborate with the interdisciplinary team to develop and follow individualized care plans
• Monitor residents for changes in condition and report concerns to charge nurse or provider
• Promote a safe, clean environment and adhere to infection control standards
• Support resident dignity and independence while fostering a warm, respectful atmosphere

Qualifications

• Active LPN license in Pennsylvania (or compact/state-appropriate) required
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable administering medications and performing routine nursing procedures
• Strong communication and teamwork skills; compassionate, patient-centered approach
• Prior experience in long-term care, skilled nursing, or similar setting preferred

Education

• LPN diploma or equivalent from an accredited nursing program
• Active Pennsylvania LPN license

Knowledge & Skills

• Medication administration and documentation best practices
• Basic clinical assessment and vital sign monitoring
• Charting and electronic health record familiarity
• Infection control, safety protocols, and resident rights
• Good interpersonal skills for working with residents, families, and care teams
