Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a long-term care setting
- Administer medications accurately and document per facility policy
- Perform routine assessments and communicate changes in patient status to the care team
- Maintain accurate charts and complete required documentation in a timely manner
- Collaborate with nurses, therapists, and support staff to deliver person-centered care
- Assist with activities of daily living while promoting dignity and independence
- Follow infection control protocols and maintain a safe environment for residents
Qualifications
- Active, unencumbered LPN license for the state of Pennsylvania
- Graduate of an accredited practical/vocational nursing program
- Current CPR/BLS certification (or obtain upon hire)
- Ability to pass background check and meet facility immunization requirements
- Strong communication skills and a team-focused attitude
- Previous long-term care experience preferred but new grads welcome to apply
Education
- Vocational/Practical Nursing diploma or equivalent required; active LPN license in PA required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic nursing assessments and resident monitoring
- Charting in electronic or paper medical records
- Infection prevention and control procedures
- Strong interpersonal skills for working with residents and families
- Time management and prioritization in a fast-paced care setting
