Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long-term care setting
• Administer medications and treatments safely and according to policy
• Monitor, assess, and document resident vitals, conditions, and responses to care
• Collaborate with interdisciplinary team members to create and follow care plans
• Communicate clearly and empathetically with residents and their families
• Maintain accurate, timely clinical documentation in the facility charting system
• Follow infection control, safety, and facility policies to protect residents and staff
• Support activities of daily living and promote resident dignity and independence

Qualifications

• Current, active LPN (Licensed Practical Nurse) license in Pennsylvania
• Completion of an accredited Practical Nursing program or vocational nursing diploma
• BLS/CPR certification required (or ability to obtain upon hire)
• Preferred: 1+ year of practical nursing experience, ideally in long-term care or skilled nursing
• Strong communication skills and a team-oriented mindset
• Ability to manage time effectively and handle multiple resident needs

Education

• Completion of an accredited Practical Nursing (LPN) program or vocational nursing diploma

Knowledge & Skills

• Medication administration and basic clinical nursing procedures
• Familiarity with wound care, infection control, and ADL assistance
• Experience with electronic medical records or charting systems
• Ability to assess changes in resident condition and escalate concerns appropriately
• Compassionate bedside manner and resident-focused care approach
• Good documentation practices and adherence to regulatory standards
