Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients with dignity and respect
• Administer medications and treatments accurately and timely, following facility protocols
• Monitor patient status, document changes clearly in the chart, and communicate concerns to the care team
• Assist with activities of daily living and ensure patient comfort and safety throughout the shift
• Collaborate with nurses, therapists and other staff to create and follow individualized care plans
• Participate in shift handoffs and interdisciplinary meetings to ensure continuity of care
• Maintain compliance with facility policies, infection control standards and regulatory requirements

Qualifications

• Current LPN (Licensed Practical Nurse) license — licensed to practice in the state of North Carolina or eligible for licensure
• Completed an accredited LPN program (vocational or practical nursing certificate/diploma)
• Basic Life Support (BLS/CPR) certification — or willing to obtain promptly
• Comfortable with medication administration, basic assessments and documentation
• Compassionate communicator with strong teamwork skills and a patient-first attitude
• Previous LPN experience in long-term care, skilled nursing, or similar settings is a plus

Education

• Completed accredited LPN (Practical Nursing) program
• High school diploma or GED

Knowledge & Skills

• Solid understanding of medication administration and infection control practices
• Proficient charting and clear clinical documentation
• Skilled in patient assessment, ADL support, and timely reporting of changes
• Strong interpersonal skills for working with residents, families and interdisciplinary teams
• Ability to prioritize tasks in a fast-paced clinical environment
