Roles & Responsibilities
- Provide compassionate, hands-on nursing care to assigned residents and patients
- Administer medications and treatments safely and accurately
- Monitor and document vital signs, changes in condition, and care given
- Assist with care planning and communicate observations to the care team
- Support wound care, ADLs, and other nursing procedures as needed
- Educate residents and families with clarity and empathy
- Maintain accurate, timely documentation in the patient record
- Follow infection control protocols to protect patients and coworkers
Qualifications
- Current Licensed Practical Nurse (LPN) license — Pennsylvania preferred/required
- Basic Life Support (BLS/CPR) certification required or obtainable upon hire
- Comfortable with medication administration and common nursing procedures
- Strong communication skills and ability to work collaboratively on a care team
- Willingness to complete facility-required health screenings and background checks
- Previous long-term care or skilled nursing experience is a plus
Education
- Completion of an accredited Practical Nursing program or vocational nursing diploma
Knowledge & Skills
- Medication administration and documentation best practices
- Fundamentals of vital signs assessment and basic clinical procedures
- EHR/clinical documentation systems familiarity
- Infection prevention and control practices
- Time management and prioritization in a busy clinical setting
- Compassionate patient-centered care and strong interpersonal skills
