Contract Type: Per-Diem

Shift Type: 8-hour shifts (specific start/end times and days to be confirmed by facility)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately in line with care plans
Monitor patient status and report changes promptly to the care team
Maintain clear, timely documentation to support continuity of care
Collaborate with nurses, providers, and support staff to deliver excellent care
Assist with activities of daily living and promote patient comfort and dignity
Participate in team huddles and contribute to a positive workplace culture

Qualifications

Current Licensed Practical Nurse (LPN) license in Pennsylvania required
Current BLS/CPR certification preferred or required as per facility policy
Comfortable administering medications and following care plans
Strong communication and teamwork skills; patient-centered mindset
Previous experience in long-term care, skilled nursing, or clinical setting preferred

Education

Diploma or certificate in Practical Nursing (LPN) or equivalent
Active Pennsylvania LPN license

Knowledge & Skills

Knowledge of routine nursing procedures and medication administration
Skill in patient assessment and recognizing changes in condition
Ability to document accurately in electronic or paper medical records
Good interpersonal skills for working with patients, families, and staff
Time management and prioritization in a fast-paced care environment
