Roles & Responsibilities
- Provide compassionate, hands-on nursing care to assigned residents and patients
- Administer medications and treatments accurately and document per policy
- Monitor patient status, report changes promptly, and escalate concerns
- Assist with activities of daily living while promoting independence and dignity
- Collaborate with the interdisciplinary team to develop and follow care plans
- Maintain clear, timely charting and support continuity of care during shift transitions
- Contribute to a positive patient and family experience through respectful communication
Qualifications
- Current LPN license in good standing (Pennsylvania license preferred or compact where applicable)
- Completion of an accredited practical nursing program or equivalent certificate
- Basic Life Support (BLS) / CPR certification or willingness to obtain promptly
- Prior experience in long-term care, rehab, or related nursing environment preferred
- Up-to-date immunizations and ability to pass background and health screenings
- Strong communication, organization, and patient-focused care skills
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED
Knowledge & Skills
- Medication administration and safe documentation practices
- Basic wound care and monitoring for changes in condition
- Familiarity with electronic medical records and accurate charting
- Ability to prioritize tasks in a fast-paced clinical setting
- Comfortable working collaboratively with nurses, aides, and providers
- Compassionate patient interaction and family communication
