Roles & Responsibilities
- Provide compassionate direct patient care and support daily living activities
- Administer medications and treatments according to policy and provider orders
- Monitor, record, and report changes in patient condition and vital signs
- Collaborate with nurses, therapists, and the care team to deliver coordinated care
- Maintain accurate clinical documentation and follow care plans
- Assist with infection control, safety protocols, and a clean care environment
- Communicate clearly and kindly with patients and their families
Qualifications
- Current LPN license in Pennsylvania (required)
- Completion of an accredited Practical Nursing program
- Current BLS/CPR certification or willingness to obtain upon hire
- Ability to pass background check and drug screen as required
- Up-to-date immunizations per facility policy
Education
- Diploma or certificate from an accredited Practical Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Solid clinical assessment and basic nursing care skills
- Medication administration and documentation best practices
- Familiarity with electronic health record systems
- Strong communication and teamwork skills
- Time management, prioritization, and ability to work in a fast-paced setting
- Knowledge of infection control and safety procedures
