Roles & Responsibilities

Provide direct nursing care to clinic patients within the scope of LPN licensure, ensuring safe, compassionate treatment
Triages patient calls and assesses urgency, directing care appropriately and escalating to providers when needed
Obtain and document accurate vital signs and clinical information for efficient provider decision-making
Manage medication refills and complete prior authorizations to keep patients’ therapies on track
Serve as the primary liaison between patients, clinical staff and providers to deliver coordinated care
Give direction to CNAs and Schedulers and collaborate with RNs, providers and clinical managers to support team workflows
Maintain accurate clinical documentation and follow current state/federal licensure and continuing education requirements

Qualifications

Current State LPN license (required)
BLS (Basic Life Support) certification (required)
Graduate of an accredited Nursing Program
Experience with triage, vital signs, medication refills and prior authorizations preferred
Comfort supervising CNAs and Schedulers and working as part of an interdisciplinary team
Ability to work weekday shifts with occasional weekend availability

Education

Graduate of a Nursing Program (LPN)

Knowledge & Skills

Triage assessment and clinical prioritization
Accurate vital signs collection and documentation
Medication refill workflows and prior authorization processes
Basic Life Support (BLS) practices
Patient-centered communication and care coordination
Team collaboration and informal supervision of support staff
