Roles & Responsibilities

• Conduct comprehensive case management assessments to identify patient needs and goals
• Coordinate multidisciplinary care and communicate effectively with physicians, social workers, and community resources
• Develop and implement individualized discharge plans to ensure safe transitions of care
• Perform utilization review and document clinical justifications clearly and professionally
• Maintain accurate and timely documentation using Midas and Meditech
• Advocate for patients and families to secure appropriate services and resources
• Collaborate with the care team to improve patient outcomes and streamline care pathways

Qualifications

• Active Registered Nurse (RN) license (must be able to practice in Florida)
• Minimum 2 years of case management experience (Experience: 2 years)
• Prior case management experience at an HCA facility is required
• Proven experience with Meditech and Midas documentation systems
• Strong clinical documentation, communication, and care coordination skills

Knowledge & Skills

• Case management best practices and discharge planning
• Meditech electronic health record proficiency
• Midas documentation system experience
• Utilization review and patient advocacy
• Excellent verbal and written communication with interdisciplinary teams
• Ability to manage a high patient ratio and prioritize effectively
