Roles & Responsibilities

• Provide occupational therapy services under the supervision of an Occupational Therapist to help residents regain independence
• Assist in developing and implementing individualized treatment plans focused on functional goals
• Conduct assessments and evaluations to identify patient needs, strengths, and realistic goals
• Administer therapeutic exercises and activities to improve motor skills, coordination, and daily living tasks
• Train patients and caregivers on use of adaptive equipment and assistive devices to maximize safety and independence
• Monitor and document patient progress clearly and promptly, adjusting plans as needed
• Collaborate with nursing and the interdisciplinary team to deliver comprehensive, person-centered care

Qualifications

• Certified Occupational Therapy Assistant (COTA) credential required
• Current state registration/licensure or eligibility to practice in Arizona
• Experience in a Skilled Nursing Facility (SNF) or geriatric/rehabilitation setting preferred
• Strong clinical documentation skills and familiarity with assessments and treatment planning
• Excellent interpersonal and teamwork abilities for interdisciplinary collaboration
• Up-to-date immunizations, background check, and ability to meet facility onboarding requirements
• BLS/CPR certification preferred

Education

• Associate degree from an accredited Occupational Therapy Assistant (OTA) program or equivalent COTA education

• Occupational Therapy principles and therapeutic interventions
• Patient assessment and evaluation techniques
• Treatment planning and goal-setting for functional outcomes
• Therapeutic exercises and activity-based interventions
• Use and training of adaptive equipment and assistive devices
