Shift Type: 4x10 Day shifts, 07:30 AM - 06:00 PM, schedule includes every other weekend (Day shift)
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Coordinate timely and safe patient discharges by collaborating with interdisciplinary teams
- Perform comprehensive needs assessments and order DME as appropriate to support transitions of care
- Develop and implement individualized plans of care focused on safe next-level placement
- Conduct utilization review and case management activities to ensure regulatory and payer requirements are met
- Document care and care plans accurately in EPIC to maintain continuity and communication
- Support a wide range of patient ages and diagnoses, applying acute care best practices across units
- Float between campuses and units as needed to meet patient and unit demands
Qualifications
- Active RN license (Pending license accepted) or MSW with relevant case management experience
- BLS certification required
- Minimum 1 year of acute care experience (Discharge Planning / Care Coordination required)
- Experience with discharge planning, care coordination, and case management/utilization review
- Comfort charting in EPIC
- Willingness to work every other weekend and float between hospital areas
Education
- Registered Nurse (RN) or Master of Social Work (MSW) — per job posting
- BLS certification required
Knowledge & Skills
- Strong knowledge of discharge planning and care coordination processes
- Experience with Needs Assessment / ordering durable medical equipment (DME)
- Familiarity with CMS rules and regulatory expectations
- Case management and utilization review skills
- Acute hospital experience across specialties (Med-Surg, ED, ICU, LLOS) is highly preferred
- Ability to manage high patient ratios (documented experience with 1:25-30 on days)
