Roles & Responsibilities

• Coordinate safe, timely patient discharges and next-level care transitions with clarity and compassion
• Perform comprehensive needs assessments and order durable medical equipment and services as needed
• Collaborate with interdisciplinary teams including social services to create and implement individualized plans of care
• Document all care coordination and utilization review activities thoroughly in EPIC
• Provide utilization review support and ensure alignment with CMS and regulatory expectations
• Float across hospital areas (including Colby and Pacific campuses) to meet patient and unit needs
• Maintain patient advocacy while balancing efficient throughput for the acute care environment

Qualifications

• Active RN licensure (RN) or MSW accepted; pending license accepted at time of submission
• Current BLS certification required
• Minimum 1 year of acute care experience required
• Demonstrated discharge planning and care coordination / case management experience
• Comfort with utilization review and CMS-related regulatory processes
• Willingness to float between hospital areas and support both Colby and Pacific campuses
• Able to work every-other-weekend rotation

Knowledge & Skills

• Proficient charting and documentation in EPIC
• Solid case management and utilization review skills
• Experience in acute care settings including medical-surgical environments
• Familiarity with ED, ICU, and long-length-of-stay (LLOS) patient populations preferred
• Strong communication and interdisciplinary collaboration skills
• Ability to manage higher patient ratios (example: day ratios listed as 1:25)
