Roles & Responsibilities

• Provide compassionate, patient-centered nursing care in a rehab/outpatient area
• Perform timely assessments and communicate changes in condition to the care team
• Administer medications and treatments safely and document according to policy
• Collaborate with therapists and providers to support patient recovery goals
• Maintain accurate, clear electronic health record documentation
• Support transitions of care with thorough handoffs and patient education
• Assist with wound care, vital signs, and routine clinical procedures as needed

Qualifications

• Current and active LPN or RN license valid in New Mexico
• Basic Life Support (BLS) certification required
• Rehab or outpatient experience preferred but not required
• Strong communication and teamwork skills
• Ability to work per diem schedule with reliability and flexibility

Education

• LPN: Graduate of an accredited practical nursing program or equivalent
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program

Knowledge & Skills

• Medication administration and monitoring
• Patient assessment and clinical documentation
• Familiarity with electronic health records and charting best practices
• Infection control and safety procedures
• Ability to work collaboratively within a multidisciplinary rehab team
