Roles & Responsibilities

• Provide compassionate direct patient care and assistance with activities of daily living
• Monitor and record patient vital signs and report changes to the care team
• Assist with safe patient transfers and mobility to promote independence
• Maintain accurate, timely documentation in patient charts
• Support rehabilitation activities under the direction of nurses and therapists
• Adhere to infection control protocols and facility policies to keep patients safe
• Communicate clearly and collaboratively with interdisciplinary team members and families

Qualifications

• Current CNA certification (required)
• T-CNA designation as noted in the posting (preferred/required where applicable)
• Basic Life Support (BLS) certification or ability to obtain per facility policy
• Up-to-date immunizations and ability to pass background and health screenings
• Comfortable working 12-hour day shifts (Wednesday-Friday)
• Reliable, compassionate, and able to work in a fast-paced rehabilitation setting

Education

• High school diploma or GED
• Completion of an accredited CNA training program

Knowledge & Skills

• Knowledge of patient care principles and ADLs
• Skill in taking and documenting vital signs accurately
• Ability to assist with transfers, positioning, and mobility
• Strong communication and teamwork skills
• Familiarity with infection control and safety practices
• Basic charting and documentation proficiency
