Roles & Responsibilities
- Provide compassionate direct patient care and monitor patient status
- Administer medications and treatments according to orders and facility policy
- Document assessments, interventions, and patient responses accurately in the chart
- Collaborate with nurses, providers, and the care team to ensure continuity of care
- Assist with care planning and communicate changes in patient condition promptly
- Promote patient safety, infection control, and a respectful environment for residents
Qualifications
- Current LPN license in Pennsylvania (required)
- Graduate of an accredited Practical Nursing program
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Comfortable working in a long-term care or similar setting; experience preferred but new grads considered
- Strong communication, critical thinking, and patient-focused care skills
Education
- Graduate of an accredited Practical Nursing (LPN) program
- Current state LPN licensure required
Knowledge & Skills
- Medication administration and monitoring
- Basic wound care and infection control practices
- Vital signs assessment and routine clinical observations
- Electronic health record documentation and charting
- Time management and ability to prioritize patient needs
- Team collaboration and clear communication with patients and families
