Roles & Responsibilities

• Provide compassionate direct patient care and daily nursing support
• Administer medications and treatments safely and accurately per orders
• Monitor and document patient conditions, reporting changes promptly
• Collaborate with RN and interdisciplinary team members to coordinate care
• Assist with admissions, discharges and patient education to support positive outcomes
• Maintain accurate, timely charting in the patient record system
• Adhere to infection control, safety protocols, and facility policies

Qualifications

• Valid LPN license (Licensed Practical Nurse) — must be active and in good standing
• Prior experience in a clinical setting preferred (acute, long-term care, or similar)
• Current BLS/CPR certification or willingness to obtain before start
• Strong communication skills and the ability to work well within a team
• Reliable, professional, and committed to patient-centered care

Education

• Graduate of an accredited Practical Nursing/Vocational Nursing program (LPN)

Knowledge & Skills

• Hands-on patient care and assessment skills
• Medication administration and documentation accuracy
• Familiarity with electronic medical records and charting best practices
• Vital signs monitoring, wound care basics, and ADL assistance
• Time management and prioritization in a fast-paced clinical setting
• Ability to follow care plans and escalate concerns appropriately
