Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments safely and accurately
- Perform timely patient assessments and report changes in condition
- Document care and maintain accurate, professional clinical records
- Collaborate with nurses, therapists, and the care team to deliver coordinated care
- Support patient and family education with clear, empathetic communication
- Adhere to facility policies, standards, and infection control practices
Qualifications
- Active Licensed Practical Nurse (LPN) license — must be valid in Pennsylvania
- Current Basic Life Support (BLS/CPR) certification (or willing to obtain)
- Comfortable with medication administration, charting, and direct patient care
- Dependable, with strong communication and teamwork skills
- Previous LPN experience in a clinical or long-term care setting preferred
Education
- Practical Nursing diploma or equivalent (LPN program completion)
Knowledge & Skills
- Proficiency in basic nursing assessments and vital sign monitoring
- Medication administration and documentation best practices
- Familiarity with electronic medical records and accurate charting
- Wound care, infection control, and safety-focused nursing
- Effective communication with patients, families, and interdisciplinary teams
- Time management and prioritization in a fast-paced care environment
