Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and on time
Monitor patient status and report changes promptly to the care team
Document care thoroughly and maintain accurate patient records
Collaborate with nurses, therapists, and care staff to deliver coordinated care
Support patient comfort, dignity, and independence through thoughtful care
Follow infection control and safety protocols consistently

Qualifications

Current LPN license in Pennsylvania (required)
Active BLS/CPR certification or willingness to obtain
Valid immunizations and TB screening per facility policy
Strong communication and teamwork skills
Preferred: prior experience in long-term care, skilled nursing, or similar settings

Education

Vocational nursing diploma or equivalent
Licensed Practical Nurse (LPN) credential

Knowledge & Skills

Patient assessment and basic clinical skills
Medication administration and documentation
Knowledge of infection control practices
Ability to follow care plans and physician orders
Comfortable using electronic health records (EHR) and basic charting
