Roles & Responsibilities
- Provide compassionate direct patient care in accordance with facility policies
- Administer medications and treatments accurately and document clearly
- Monitor patient status and report changes to the care team promptly
- Collaborate with nurses and interdisciplinary staff to support care plans
- Maintain accurate and timely documentation of patient assessments and interventions
- Support a safe, clean, and therapeutic environment for residents
Qualifications
- Valid LPN license (practical nurse) — active and in good standing
- Current Basic Life Support (BLS) certification or willing to obtain
- Comfortable working 12.5-hour shifts and delivering hands-on patient care
- Strong communication skills and a team-oriented attitude
- Prior experience in long-term care, skilled nursing, or similar settings preferred
- Ability to pass required background checks and facility onboarding
Education
- Completion of an accredited LPN/LVN program
- High school diploma or GED required; additional coursework a plus
Knowledge & Skills
- Clinical assessment and basic nursing interventions
- Medication administration and documentation
- Infection control and safety protocols
- Time management and ability to prioritize patient needs
- Effective communication with patients, families, and care teams
- Familiarity with electronic charting or willingness to learn facility systems
