Roles & Responsibilities

• Deliver compassionate nursing care to residents, focusing on safety and dignity
• Administer medications and treatments accurately and in a timely manner
• Assess residents’ conditions and communicate changes to the interdisciplinary team
• Document care thoroughly in the medical record to ensure continuity and quality
• Assist with activities of daily living and support residents’ comfort and mobility
• Collaborate with nurses, therapists, and support staff to create person-centered plans
• Maintain infection control standards and a safe therapeutic environment
• Support residents and families with clear, empathetic communication

Qualifications

• Current LPN license (Licensed Practical Nurse) — ready to practice
• Basic Life Support (BLS) / CPR certification preferred or required per facility policy
• Comfortable working in long-term care or skilled nursing settings
• Strong communication skills and ability to work as part of a multi-disciplinary team
• Reliable, punctual, and committed to resident-centered care

Education

• Graduate of an accredited LPN program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment and basic nursing interventions
• Familiarity with infection prevention and safety protocols
• Ability to use electronic medical records (EMR) or willingness to learn
• Time management and prioritization in a fast-paced care environment
• Team collaboration and clear handoff/reporting skills
