Roles & Responsibilities

• Provide compassionate, hands-on care to residents and patients, helping with activities of daily living like bathing, dressing, and feeding
• Assist nursing staff with safe transfers and mobility support to prevent falls and injuries
• Monitor and report changes in patient condition to the RN/LPN promptly and accurately
• Perform routine tasks such as bathing, toileting, grooming, and basic comfort measures with dignity and respect
• Document care and observations clearly and timely in the patient record
• Maintain a clean, safe environment by following infection control and safety protocols
• Participate in orientation and ongoing training to stay current with facility practices

Qualifications

• Current LNA certification valid in New Hampshire
• High school diploma or equivalent
• Basic Life Support (BLS/CPR) certification preferred or willing to obtain
• Ability to pass background check and meet facility immunization requirements
• Comfortable with physical aspects of the role, including lifting and assisting patients
• Reliable, punctual, and able to work flexible per diem shifts

Education

• High school diploma or GED and active LNA certification

Knowledge & Skills

• Strong interpersonal and communication skills to interact with patients and the care team
• Knowledge of basic patient care procedures and infection prevention
• Ability to follow care plans and document accurately
• Team player mindset with a positive, patient-centered attitude
• Good time management and the ability to prioritize tasks in a busy clinical setting
