Shift Type: Variable days (specific days to be confirmed with facility), 3:00 PM - 11:00 PM Evening shift; every other weekend required; local-only assignment

Hours per Week: 37

Candidate Pay Rate: $21 per hour

Roles & Responsibilities

Provide compassionate personal care and assist patients with activities of daily living
Monitor patient condition and promptly report changes to the nursing team
Perform routine clinical tasks such as bathing, dressing, feeding, and mobility assistance
Accurately document care, observations, and patient interactions in the chart
Collaborate with nurses, therapists, and support staff to create a safe, respectful environment
Support infection control and safety protocols to protect patients and colleagues

Qualifications

Current Licensed Nursing Assistant (LNA) certification (VT/NH accepted) required
Reliable transportation and availability for a local-only assignment
Ability to work the 3:00 PM - 11:00 PM evening shift and every other weekend
Strong communication skills and a caring, patient-centered attitude
Previous long-term care or rehabilitation experience is a plus

Education

High school diploma or equivalent preferred
Licensed Nursing Assistant (LNA) certification required

Knowledge & Skills

Proficient in basic patient care, ADL assistance, and mobility support
Comfortable taking and recording vital signs and observing clinical changes
Familiarity with documentation best practices and basic charting
Good time management and ability to prioritize patient needs
Team-oriented mindset and strong patient communication skills
