Roles & Responsibilities

• Assess patients' nutritional status and create individualized, measurable nutrition care plans
• Provide nutrition education and counseling to patients, families, and caregivers tailored to cultural and personal preferences
• Coordinate with diet clerks and nursing units to ensure accurate diets are served and diet order reconciliation is completed
• Document assessments, interventions, education, and care plans in the medical record following approved formats
• Make clinical rounds to identify patients at nutritional risk and prioritize care through interdisciplinary screening
• Monitor patient tolerance to therapeutic diets and recommend adjustments as needed
• Contact physicians proactively about questions or irregularities in nutritional care
• Contribute to the development and monitoring of departmental clinical performance indicators

Qualifications

• DCI/BVP certification required
• License/Certificate verification required (current and valid)
• Completion of Prime Core Competencies Exam and Prime Skills Checklist as part of onboarding
• Ability to perform timely and accurate nutritional assessments and diagnoses
• Comfort collaborating with interdisciplinary teams including nursing, pharmacy, social services, and medical staff
• Willingness to complete required health screenings and immunizations
