Roles & Responsibilities

• Provide compassionate, hands-on care to patients including assistance with activities of daily living
• Assist nursing staff with patient transfers, bathing, dressing, and toileting
• Monitor and record vital signs and report changes in patient condition promptly
• Document care activities accurately in the patient chart or electronic health record
• Communicate clearly and respectfully with patients, families, and the care team
• Support a safe, clean environment by following infection control and safety protocols
• Contribute to a positive patient experience by offering comfort, respect, and timely assistance

Qualifications

• Active CNA certification (or equivalent) — Pennsylvania certification preferred
• Current CPR/BLS certification or willingness to obtain
• Ability to pass required background and drug screening
• Reliable transportation and ability to respond to per-diem shift needs
• Comfortable working in a fast-paced team environment with flexibility to cover varying shifts
• Previous experience in long-term care or skilled nursing settings is a plus

Education

• High school diploma or equivalent
• Completion of an accredited CNA program

Knowledge & Skills

• Strong patient care skills including ADLs and mobility assistance
• Basic vital signs assessment and observation skills
• Familiarity with infection control best practices
• Clear verbal and written communication
• Ability to follow care plans and document accurately in charts or EHR
• Teamwork, time management, and a calm bedside manner
