Roles & Responsibilities

Provide compassionate daily care and assistance with activities of daily living
Assist patients with mobility, transfers, and positioning while prioritizing safety
Take and record vital signs accurately and report changes to the nursing team
Document patient care activities clearly in the chart or EMR
Support a clean, safe and respectful environment for residents and patients
Communicate effectively with nurses, therapists and family members to coordinate care
Participate in care planning and help implement individualized care tasks

Qualifications

Current Certified Nursing Assistant (CNA) certification in Pennsylvania
CPR/BLS certification preferred or willingness to obtain
Ability to pass background check and required health screenings
Compassionate, patient-focused attitude with good communication skills
Previous experience in long-term care or assisted living is a plus

Education

High school diploma or equivalent and completion of an accredited CNA program

Knowledge & Skills

Basic nursing care and patient assistance (ADLs)
Taking and recording vital signs
Infection control and safe patient handling
Clear documentation and basic EMR familiarity
Strong teamwork, time management, and communication skills
