Roles & Responsibilities
- Provide compassionate, evidence-based nursing care to PACE and long-term care participants
- Conduct thorough assessments, monitor patient status, and document care accurately
- Administer medications safely and manage medication reconciliation
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Educate participants and families about health conditions and care plans to promote self-management
- Participate in infection control, immunization verification, and required health screenings
- Support smooth transitions of care between settings and communicate changes promptly
Qualifications
- Active, unencumbered RN license in Louisiana (or compact RN license as applicable)
- Current Basic Life Support (BLS) certification; additional certifications (e.g., ACLS) a plus
- Experience or interest in long-term care, PACE programs, or geriatric nursing preferred
- Up-to-date immunizations per employer requirements (MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza)
- Ability to pass Trinity 10 Panel Drug Screen and required health screenings
- Strong communication skills and a team-oriented mindset
Education
- Graduate of an accredited nursing program (ADN or BSN)
Knowledge & Skills
- Familiarity with long-term care and PACE program workflows and best practices
- Skilled in patient assessment, care planning, and medication administration
- Proficient charting and documentation with attention to detail
- Knowledge of infection prevention and immunization requirements
- Ability to triage, prioritize, and escalate changes in patient condition
- Comfort working independently in per diem roles and collaborating across disciplines
