Roles & Responsibilities

• Provide compassionate direct nursing care to residents in a long-term care setting
• Administer medications and treatments safely and accurately according to policies
• Develop, update, and implement individualized care plans in collaboration with the care team
• Monitor resident condition, recognize changes, and escalate concerns promptly
• Document care and assessments clearly and timely in the electronic medical record
• Support infection control practices and maintain a safe, therapeutic environment
• Communicate effectively with residents, families, and interdisciplinary staff to promote quality of life
• Assist with admissions, transfers, and discharge activities as needed

Qualifications

• Active Registered Nurse (RN) license in New Jersey (required)
• BLS (Basic Life Support) certification required; ACLS preferred if applicable
• Immunizations and health requirements: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, annual influenza vaccine, completed physical/health statement
• Successful Trinity 10 Panel Drug Screen (per facility requirement)
• Experience in long-term care or skilled nursing preferred; per diem or pool experience a plus
• Strong clinical assessment, medication administration, and documentation skills
• Professional, dependable, and able to adapt to variable schedules

Education

• Graduate of an accredited nursing program (ADN or BSN)
• BSN preferred but ADN with experience accepted

Knowledge & Skills

• Thorough knowledge of long-term care nursing practices and resident-focused care
• Medication administration and IV/PO documentation accuracy
• Familiarity with electronic medical records and charting best practices
• Skilled in wound care, ADL support, and chronic disease monitoring
• Excellent communication, teamwork, and time management skills
• Ability to prioritize, triage, and respond to urgent resident needs calmly
