Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in a long-term care and rehabilitation setting
- Perform thorough assessments and develop or update individualized care plans
- Administer medications and treatments accurately and in line with facility protocols
- Collaborate with therapists, social workers, and other clinicians to coordinate care and support resident goals
- Document clinical findings and interventions clearly and promptly in the medical record
- Participate in infection control, safety, and quality improvement initiatives
- Communicate effectively with families and caregivers to support resident well-being
- Support and mentor CNAs and other care staff as needed to maintain high standards
Qualifications
- Current Registered Nurse (RN) license — ability to practice in Indiana or eligibility through compact licensure
- BLS (Basic Life Support) certification required
- Experience working in Long Term Care (LTC) or skilled nursing/rehabilitation settings preferred
- Completion of required health screenings and immunizations (see site requirements)
- Successful completion of facility-required drug screening, fit testing, and health documentation
- Strong clinical judgment, reliable attendance, and a patient-first attitude
Knowledge & Skills
- Knowledge of long-term care nursing standards and regulatory requirements
- Proficiency in medication administration and basic wound care
- Familiarity with infection prevention and isolation procedures
- Comfortable using electronic medical records and accurate documentation practices
- Strong communication and teamwork skills with residents, families, and staff
- Time management and prioritization in a fast-paced clinical environment
