Roles & Responsibilities
- Provide compassionate, high-quality nursing care to long-term care residents with dignity and respect
- Perform resident assessments, develop and update individualized care plans, and document thoroughly
- Administer medications and treatments safely according to policy and physician orders
- Collaborate with nursing staff, therapists, and interdisciplinary team members to optimize resident outcomes
- Monitor resident status, recognize changes, and escalate concerns in a timely manner
- Support transitions of care and communicate effectively with residents and families
Qualifications
- Active Registered Nurse (RN) license (license verification required)
- Current BLS and ACLS certifications
- Experience or interest in Long-Term Care (LTC) nursing and working in a resident-focused environment
- Completion of required health screening: 2-step PPD or IGRA (Quantiferon), MMR, Hepatitis B, Varicella, Tdap, COVID-19 and influenza vaccination as required
- Successful completion of Trinity-specific competency testing, skills checklist, color vision screening, and 10-panel drug screen
- Ability to pass physical/health statement and meet facility onboarding requirements
Knowledge & Skills
- Clinical assessment and care planning tailored to long-term care populations
- Medication administration and documentation accuracy
- Infection prevention and control practices
- Strong communication, teamwork, and resident/family-centered care
- Familiarity with LTC regulations and electronic medical record documentation
- Ability to adapt quickly across different shift patterns and facility workflows
