Roles & Responsibilities

• Provide compassionate, high-quality nursing care to long-term care residents with dignity and respect
• Perform resident assessments, develop and update individualized care plans, and document thoroughly
• Administer medications and treatments safely according to policy and physician orders
• Collaborate with nursing staff, therapists, and interdisciplinary team members to optimize resident outcomes
• Monitor resident status, recognize changes, and escalate concerns in a timely manner
• Support transitions of care and communicate effectively with residents and families

Qualifications

• Active Registered Nurse (RN) license (license verification required)
• Current BLS and ACLS certifications
• Experience or interest in Long-Term Care (LTC) nursing and working in a resident-focused environment
• Completion of required health screening: 2-step PPD or IGRA (Quantiferon), MMR, Hepatitis B, Varicella, Tdap, COVID-19 and influenza vaccination as required
• Successful completion of Trinity-specific competency testing, skills checklist, color vision screening, and 10-panel drug screen
• Ability to pass physical/health statement and meet facility onboarding requirements

Knowledge & Skills

• Clinical assessment and care planning tailored to long-term care populations
• Medication administration and documentation accuracy
• Infection prevention and control practices
• Strong communication, teamwork, and resident/family-centered care
• Familiarity with LTC regulations and electronic medical record documentation
• Ability to adapt quickly across different shift patterns and facility workflows
