Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a skilled nursing/long-term care environment
- Perform thorough assessments, develop nursing plans, and document care accurately
- Administer medications and treatments safely and according to provider orders
- Collaborate with an interdisciplinary team to optimize resident outcomes
- Support infection control practices and complete required health and competency screenings
- Participate in Trinity specialty competency testing and maintain required certifications
- Communicate clearly with residents, families, and colleagues to promote continuity of care
Qualifications
- Current Registered Nurse (RN) license (license verification required)
- BLS certification required
- Experience or comfort working in Skilled Nursing Facility (SNF) or Long Term Care (LTC) settings
- Successful completion of Trinity specialty competency testing as required
- Completion of required health screenings and vaccinations (fit test, TB screening or IGRA, influenza, MMR, Hepatitis B, varicella, Tdap, COVID-19, drug screen, physical/health statement)
- Ability to pass Trinity respirator evaluation and Trinity color vision checks
Knowledge & Skills
- Strong clinical assessment and documentation skills for the long-term care population
- Medication administration and IV/PO management knowledge
- Familiarity with infection prevention protocols and resident safety practices
- Good communication and teamwork skills for interdisciplinary collaboration
- Comfort with electronic medical records and timely charting
- Ability to adapt to varied assignments and provide high-quality care across shifts
