Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a skilled nursing/long-term care environment
• Perform thorough assessments, develop nursing plans, and document care accurately
• Administer medications and treatments safely and according to provider orders
• Collaborate with an interdisciplinary team to optimize resident outcomes
• Support infection control practices and complete required health and competency screenings
• Participate in Trinity specialty competency testing and maintain required certifications
• Communicate clearly with residents, families, and colleagues to promote continuity of care

Qualifications

• Current Registered Nurse (RN) license (license verification required)
• BLS certification required
• Experience or comfort working in Skilled Nursing Facility (SNF) or Long Term Care (LTC) settings
• Successful completion of Trinity specialty competency testing as required
• Completion of required health screenings and vaccinations (fit test, TB screening or IGRA, influenza, MMR, Hepatitis B, varicella, Tdap, COVID-19, drug screen, physical/health statement)
• Ability to pass Trinity respirator evaluation and Trinity color vision checks

Knowledge & Skills

• Strong clinical assessment and documentation skills for the long-term care population
• Medication administration and IV/PO management knowledge
• Familiarity with infection prevention protocols and resident safety practices
• Good communication and teamwork skills for interdisciplinary collaboration
• Comfort with electronic medical records and timely charting
• Ability to adapt to varied assignments and provide high-quality care across shifts
