Roles & Responsibilities

• Provide compassionate direct patient care in patients' homes, assisting with daily living activities
• Assist with bathing, dressing, grooming, mobility and basic hygiene needs
• Monitor and report changes in patient condition to the supervising nurse
• Perform vital signs, basic clinical tasks, and documentation accurately and timely
• Support safe infection control practices and follow care plans
• Communicate clearly and respectfully with patients, families, and the care team
• Complete required competency testing and training to maintain high-quality care
• Travel safely to patient homes and represent Mercy Home Health professionally

Qualifications

• Current Certified Nursing Assistant (CNA) certification (required)
• Basic Life Support (BLS) certification (required)
• Successful License/Certificate Verification as part of onboarding
• Completion of Trinity Specialty Specific Competency Testing as required
• Ability to pass Trinity 10 Panel Drug Screen, fit test, and required immunizations
• Comfort working independently in patients' homes and strong customer service skills
• Home health or patient care experience preferred but caring attitude is essential

Education

• Certified Nursing Assistant (CNA) certification (required)
• High school diploma or GED preferred

Knowledge & Skills

• Knowledge of basic patient care procedures and activities of daily living
• Strong infection control and safety practice awareness
• Clear verbal and written communication for documentation and family updates
• Time management and ability to prioritize patient needs
• Detail-oriented with accurate charting and reporting skills
• Professionalism when representing Mercy Home Health in community settings
