Roles & Responsibilities
- Provide compassionate direct patient care in patients' homes, assisting with daily living activities
- Assist with bathing, dressing, grooming, mobility and basic hygiene needs
- Monitor and report changes in patient condition to the supervising nurse
- Perform vital signs, basic clinical tasks, and documentation accurately and timely
- Support safe infection control practices and follow care plans
- Communicate clearly and respectfully with patients, families, and the care team
- Complete required competency testing and training to maintain high-quality care
- Travel safely to patient homes and represent Mercy Home Health professionally
Qualifications
- Current Certified Nursing Assistant (CNA) certification (required)
- Basic Life Support (BLS) certification (required)
- Successful License/Certificate Verification as part of onboarding
- Completion of Trinity Specialty Specific Competency Testing as required
- Ability to pass Trinity 10 Panel Drug Screen, fit test, and required immunizations
- Comfort working independently in patients' homes and strong customer service skills
- Home health or patient care experience preferred but caring attitude is essential
Education
- Certified Nursing Assistant (CNA) certification (required)
- High school diploma or GED preferred
Knowledge & Skills
- Knowledge of basic patient care procedures and activities of daily living
- Strong infection control and safety practice awareness
- Clear verbal and written communication for documentation and family updates
- Time management and ability to prioritize patient needs
- Detail-oriented with accurate charting and reporting skills
- Professionalism when representing Mercy Home Health in community settings
