Roles & Responsibilities

• Provide compassionate personal care and assist residents with activities of daily living
• Take and document vital signs, reporting any changes to nursing staff
• Assist with mobility, transfers, and safe patient handling
• Maintain accurate and timely documentation in accordance with facility policies
• Adhere to infection control practices and facility vaccination requirements
• Complete Trinity Specialty Specific Competency Testing and the Trinity Skills Checklist
• Participate in license and certification verification and maintain required health screenings
• Collaborate with the interdisciplinary team to support resident-centered care

Qualifications

• Valid CNA certification in the state of Pennsylvania (required)
• Current BLS certification (required)
• Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• License/certificate verification and required health screenings (2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine)
• Successful Trinity 10 panel drug screen
• Reliable, flexible, and able to work per diem pool assignments including weekends and varied shifts

Education

• CNA certification (required)
• High school diploma or equivalent (preferred)

Knowledge & Skills

• Strong understanding of patient safety and infection prevention
• Proficiency in assisting with ADLs and resident hygiene
• Accurate vital signs measurement and documentation
• Effective communication and teamwork skills
• Ability to follow facility policies, competency checklists, and clinical protocols
• Comfort with routine health screenings and compliance procedures
