Roles & Responsibilities
- Provide compassionate personal care and assist residents with activities of daily living
- Take and document vital signs, reporting any changes to nursing staff
- Assist with mobility, transfers, and safe patient handling
- Maintain accurate and timely documentation in accordance with facility policies
- Adhere to infection control practices and facility vaccination requirements
- Complete Trinity Specialty Specific Competency Testing and the Trinity Skills Checklist
- Participate in license and certification verification and maintain required health screenings
- Collaborate with the interdisciplinary team to support resident-centered care
Qualifications
- Valid CNA certification in the state of Pennsylvania (required)
- Current BLS certification (required)
- Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
- License/certificate verification and required health screenings (2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine)
- Successful Trinity 10 panel drug screen
- Reliable, flexible, and able to work per diem pool assignments including weekends and varied shifts
Education
- CNA certification (required)
- High school diploma or equivalent (preferred)
Knowledge & Skills
- Strong understanding of patient safety and infection prevention
- Proficiency in assisting with ADLs and resident hygiene
- Accurate vital signs measurement and documentation
- Effective communication and teamwork skills
- Ability to follow facility policies, competency checklists, and clinical protocols
- Comfort with routine health screenings and compliance procedures
