Roles & Responsibilities
- Provide compassionate direct care to residents as assigned, helping with activities of daily living
- Monitor and record vital signs and communicate changes to the care team
- Assist with mobility, transfers, and fall prevention to keep residents safe
- Support meal assistance and feeding while promoting resident dignity
- Follow care plans and document care accurately and timely
- Participate in competency testing and skills checklists to maintain high standards
- Adhere to infection control practices and facility policies for a safe environment
- Collaborate with nurses and interdisciplinary staff to deliver consistent, quality care
Qualifications
- Current CNA certification in good standing
- Basic Life Support (BLS) certification required
- License or certificate verification completed and up to date
- Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist as required
- Current Trinity 10 Panel Drug Screen and negative background as required
- Up-to-date immunizations or proof of immunity: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza
- 2-Step PPD or IGRA (Quantiferon) for TB screening
- Physical/Health Statement confirming fitness for duty
Education
- High school diploma or equivalent preferred
- Completion of an approved CNA training program and active CNA certification
Knowledge & Skills
- Strong patient-focused caregiving and bedside manner
- Basic clinical skills: vital signs, positioning, feeding, hygiene assistance
- Clear communication with residents, families, and clinical teams
- Reliable documentation and charting of provided care
- Understanding of infection prevention and standard precautions
- Ability to follow care plans and complete competency assessments
- Comfort working in a per diem pool with changing assignments and locations
