Roles & Responsibilities

• Assess and monitor patient condition, promptly recognizing any changes and escalating concerns
• Deliver high-quality nursing care including medication administration, wound care, and clinical treatments
• Perform standardized stroke assessments using NIHSS when applicable and document findings accurately
• Maintain clear, timely documentation in the patient record and support safe handoffs
• Collaborate with physicians, therapists, and the interdisciplinary team to create and follow individualized care plans
• Educate patients and families about diagnoses, treatments, discharge plans, and follow-up care
• Adhere to facility policies and complete required Trinity competency checklists and specialty testing

Qualifications

• Current, active RN license in Illinois (license/certificate verification required)
• BLS certification required
• NIHSS certification required (for stroke assessment competency)
• Completion of Trinity Skills Checklist and Trinity specialty-specific competency testing
• Successful Trinity 10-panel drug screen, Fit Test, and Trinity Color Vision check
• Up-to-date immunizations and health documentation: COVID-19 vaccine, MMR, Hepatitis B, Influenza vaccine, Varicella, Tdap, and a current Physical/Health Statement

Knowledge & Skills

• Strong patient assessment and clinical monitoring skills
• Experience with stroke assessment protocols (NIHSS) and neurologic observation
• Effective clinical documentation and EHR proficiency
• Infection control and safety-focused practice
• Excellent communication and teamwork with interdisciplinary staff
• Ability to follow competency checklists and complete required testing accurately
