Roles & Responsibilities

• Perform and document patient assessments appropriate to LPN licensure
• Complete admission processes and ensure accurate health history intake
• Provide patient care and personal cares with compassion and professionalism
• Identify discharge planning and education needs, and help prepare patients/families
• Implement nursing interventions and report early warning signs of condition changes
• Collaborate and communicate timely patient information with the multidisciplinary team
• Delegate and assign tasks safely to team members based on competence
• Contribute to evaluation and modification of the plan of care in partnership with licensed practitioners

Qualifications

• Active MN LPN Licensure (required)
• Current Licensed Practical Nurse (LPN) credential
• Demonstrated ability to perform patient assessment and complete timely documentation
• Strong communication skills for working with patients, families, and multidisciplinary teams
• Ability to implement nursing interventions and follow prescribed medical regimens
• Comfort with delegation and working within defined standards of care
