Roles & Responsibilities
- Evaluate patient functional status and develop individualized PT treatment plans
- Provide hands-on therapy and therapeutic exercise to maximize mobility and independence
- Document patient progress clearly and accurately in the medical record
- Collaborate with nursing, OT, and other care team members to coordinate care
- Educate patients and caregivers on home programs, safety, and injury prevention
- Monitor outcomes and adjust treatment plans to meet patient goals and facility standards
- Adhere to facility policies, safety protocols, and professional ethical standards
Qualifications
- Valid license as a Physical Therapist in California or eligibility to obtain one
- Current Basic Life Support (BLS) / CPR certification (or willing to obtain)
- Clinical experience delivering physical therapy in outpatient, inpatient, or correctional settings preferred
- Strong documentation skills and familiarity with Medicare/Medicaid documentation practices
- Excellent communication skills and ability to work within an interdisciplinary team
- Ability to pass facility background checks and meet site-specific credentialing
Education
- Doctor of Physical Therapy (DPT) preferred; entry-level Master’s or Bachelor's in Physical Therapy accepted where applicable
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise, manual therapy, and functional training
- Knowledge of standardized outcome measures and goal-oriented care planning
- Proficient with clinical documentation and electronic health record systems
- Strong clinical reasoning, time management, and patient-centered care approach
- Ability to adapt interventions for a diverse patient population and unique facility environment
