Roles & Responsibilities

• Evaluate patient functional status and develop individualized PT treatment plans
• Provide hands-on therapy and therapeutic exercise to maximize mobility and independence
• Document patient progress clearly and accurately in the medical record
• Collaborate with nursing, OT, and other care team members to coordinate care
• Educate patients and caregivers on home programs, safety, and injury prevention
• Monitor outcomes and adjust treatment plans to meet patient goals and facility standards
• Adhere to facility policies, safety protocols, and professional ethical standards

Qualifications

• Valid license as a Physical Therapist in California or eligibility to obtain one
• Current Basic Life Support (BLS) / CPR certification (or willing to obtain)
• Clinical experience delivering physical therapy in outpatient, inpatient, or correctional settings preferred
• Strong documentation skills and familiarity with Medicare/Medicaid documentation practices
• Excellent communication skills and ability to work within an interdisciplinary team
• Ability to pass facility background checks and meet site-specific credentialing

Education

• Doctor of Physical Therapy (DPT) preferred; entry-level Master’s or Bachelor's in Physical Therapy accepted where applicable

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, manual therapy, and functional training
• Knowledge of standardized outcome measures and goal-oriented care planning
• Proficient with clinical documentation and electronic health record systems
• Strong clinical reasoning, time management, and patient-centered care approach
• Ability to adapt interventions for a diverse patient population and unique facility environment
