Roles & Responsibilities

• Deliver patient-centered physical therapy evaluations and evidence-based treatment plans
• Implement therapeutic interventions that restore mobility, reduce pain, and improve function
• Collaborate with nurses, physicians, and rehab team members to coordinate care
• Document patient progress clearly and timely in the medical record
• Educate patients and families on home programs, safety, and long-term goals
• Participate in discharge planning to ensure safe transitions and continued recovery
• Contribute to a positive, professional culture that supports patient dignity and staff teamwork

Qualifications

• Licensed Physical Therapist in California or ability to obtain state license prior to start
• Active BLS/CPR certification (or willingness to obtain upon hire)
• Experience with outpatient and post-acute rehabilitation preferred
• Strong clinical reasoning and ability to create individualized treatment plans
• Excellent communication skills and a team-first attitude
• Ability to pass required background checks and compliance screenings

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy, and functional mobility training
• Familiarity with documentation standards and electronic medical records
• Understanding of discharge planning and durable medical equipment needs
• Ability to educate patients, families, and caregivers in a clear, compassionate way
• Time management and caseload prioritization for efficient, high-quality care
