Shift Type: Monday-Friday, 8:00 AM - 4:00 PM Day (30-minute meal break)

Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Conduct comprehensive psychosocial assessments to understand each resident's needs
- Develop and implement individualized care and discharge plans in collaboration with the care team
- Provide compassionate counseling, crisis intervention, and resource linkage for patients and families
- Coordinate with interdisciplinary teams, external providers, and community resources to ensure continuity of care
- Document clinical interactions, progress notes, and care plans accurately and timely
- Advocate for patient-centered care and support quality improvement initiatives at the facility
Qualifications
- Licensed Social Worker in the state of practice or ability to obtain state licensure prior to start
- BSW or MSW degree (see education for details)
- Experience in clinical social work, case management, discharge planning, or related settings
- Strong communication and documentation skills with attention to detail
- Able to pass background checks and meet facility onboarding requirements
Education
- BSW (Bachelor of Social Work) required
- MSW (Master of Social Work) preferred
Knowledge & Skills
- Skilled in psychosocial assessment, care planning, and crisis intervention
- Knowledgeable about community resources, discharge planning, and care coordination
- Comfortable working within an interdisciplinary team and advocating for patients
- Proficient with electronic medical records and clinical documentation
- Strong interpersonal skills and the ability to build rapport with patients and families
