Roles & Responsibilities

• Conduct comprehensive psychosocial assessments to understand each resident's needs
• Develop and implement individualized care and discharge plans in collaboration with the care team
• Provide compassionate counseling, crisis intervention, and resource linkage for patients and families
• Coordinate with interdisciplinary teams, external providers, and community resources to ensure continuity of care
• Document clinical interactions, progress notes, and care plans accurately and timely
• Advocate for patient-centered care and support quality improvement initiatives at the facility

Qualifications

• Licensed Social Worker in the state of practice or ability to obtain state licensure prior to start
• BSW or MSW degree (see education for details)
• Experience in clinical social work, case management, discharge planning, or related settings
• Strong communication and documentation skills with attention to detail
• Able to pass background checks and meet facility onboarding requirements

Education

• BSW (Bachelor of Social Work) required
• MSW (Master of Social Work) preferred

Knowledge & Skills

• Skilled in psychosocial assessment, care planning, and crisis intervention
• Knowledgeable about community resources, discharge planning, and care coordination
• Comfortable working within an interdisciplinary team and advocating for patients
• Proficient with electronic medical records and clinical documentation
• Strong interpersonal skills and the ability to build rapport with patients and families
