Roles & Responsibilities

• Provide compassionate, evidence based nursing care to patients in a family practice rural health clinic
• Perform thorough patient assessments, triage, and vitals to guide care plans and provider decisions
• Administer medications and immunizations safely and document according to clinic policies
• Support chronic disease management with patient education, follow up, and care coordination
• Assist providers with procedures, wound care, and point of care testing
• Maintain accurate and timely electronic medical records and facilitate referrals
• Collaborate with interdisciplinary team members to improve patient outcomes and clinic workflow
• Act as an advocate for patients, connecting them to community resources and preventive care

Qualifications

• Active Registered Nurse (RN) license in Wisconsin or eligibility to obtain one promptly
• Current BLS certification required; ACLS or PALS a plus depending on clinic needs
• Comfortable and experienced working in outpatient or family practice settings, especially rural clinics
• Strong clinical assessment, triage, and patient education skills
• Reliable, professional, and able to work autonomously while collaborating with the care team
• Ability to pass background checks and meet credentialing requirements for a travel contract

Education

• Graduate of an accredited nursing program (ADN or BSN)
• BSN preferred but not required

Knowledge & Skills

• Family practice clinical workflows and outpatient nursing procedures
• Chronic disease management including diabetes, hypertension and COPD basics
• Immunization administration and documentation best practices
• Proficiency with electronic health records and timely charting
• Patient education, motivational interviewing and preventive care counseling
• Triage, vital signs interpretation and point of care testing
