Roles & Responsibilities

• Perform comprehensive home health assessments including Admission, Start of Care, Discharge, Recertification and Resumption of Care
• Deliver hands-on skilled care: wound care, medication administration (including IM, IV, subcutaneous), PCA pump management and IV infusions
• Document thoroughly and accurately in EPIC and complete required OASIS documentation for home health outcomes
• Assess and manage patient pain using appropriate scales and interventions, and educate patients and caregivers on pain management
• Apply infection control precautions and ensure safe care environments in patients’ homes
• Coordinate care and communicate with interdisciplinary team members to support safe discharges and continuity of care
• Maintain professional standards and BCLS certification, and comply with facility vaccination requirements

Qualifications

• Active RN license (Oregon) — pending licenses are accepted; please disclose status up front
• Minimum 1 year of skilled adult Home Health nursing experience required — travelers with prior home health experience only (no first-time travelers)
• Current BCLS/BLS certification required
• OASIS experience required — please call out if you do not have OASIS experience
• Comfort with EPIC charting and computerized documentation
• Own reliable transportation and ability to travel within a service radius (occasional max radius 50+ miles)
• Facility requirement: completed primary COVID-19 vaccination series plus one booster

• Home Health Nursing and patient-centered care

• OASIS documentation and EPIC charting proficiency
• Wound care techniques and integumentary assessment
• Medication administration: oral, intradermal, IM, subcutaneous and intravenous infusions
• PCA pump monitoring and pain assessment/management
• Infection control precautions and safe home-based practices
• Case management fundamentals and working within interdisciplinary teams
