Roles & Responsibilities
- Evaluate patients and design individualized rehabilitation plans to restore function and mobility
- Deliver evidence-based physical therapy interventions with compassion and clinical precision
- Monitor patient progress and adjust plans of care to achieve measurable outcomes
- Educate patients and families on exercises, safety, and recovery to maximize independence
- Maintain accurate and timely clinical documentation for continuity and quality of care
- Collaborate closely with physicians and interdisciplinary teams to coordinate comprehensive treatment
Qualifications
- Active Physical Therapist license in New Hampshire (required)
- Degree in Physical Therapy from an accredited program (e.g., DPT)
- Rehab or outpatient experience preferred and highly valued
- Strong clinical judgment, patient-centered approach, and commitment to quality care
- Excellent communication skills and reliable clinical documentation practices
- Ability to work collaboratively within an interdisciplinary care team
Education
- Degree in Physical Therapy from an accredited program (e.g., Doctor of Physical Therapy - DPT)
Knowledge & Skills
- Clinical assessment and treatment planning for rehabilitation patients
- Evidence-based physical therapy interventions and manual techniques
- Progress monitoring and outcome-driven plan adjustments
- Patient and family education methods to support recovery and safety
- Electronic clinical documentation systems and timely charting
- Interdisciplinary communication and care coordination
