Roles & Responsibilities
- Evaluate patients in the home setting and develop individualized, evidence-based treatment plans
- Provide skilled physical therapy services to improve mobility, strength, balance, and functional independence
- Educate patients and caregivers on exercises, safety strategies, and injury prevention in a clear, compassionate way
- Document evaluations, progress notes, and outcomes accurately and in a timely manner
- Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate high-quality care
Qualifications
- Active Physical Therapist (PT) license in the state of Iowa
- Graduate of an accredited Physical Therapy program
- Home Health experience preferred but not required — we welcome motivated clinicians ready to learn
- Strong clinical judgment, time management, and communication skills
- Ability to work independently while delivering compassionate, patient-centered care
Education
- Graduate of an accredited Physical Therapy program
