Candidate Pay Rate: $63 per hour

Roles & Responsibilities
- Complete OASIS documentation accurately and on time to support quality care and compliance
- Assess patient and caregiver needs using the nursing process to create individualized plans of care
- Develop and communicate realistic, evidence-based plans of care that include patients and caregivers
- Serve as case manager for assigned patients, managing caseloads and visit schedules
- Implement nursing interventions and procedures to ensure safe, effective patient care
- Perform high-tech nursing skills including IV use and maintenance, wound VAC management, and complex wound care
- Maintain infection control standards across the plan of care
- Collaborate with LPNs and interdisciplinary team members and delegate within scope
- Provide patient and caregiver education on medications, treatments, discharge instructions, and reportable conditions
- Complete agency, state and federal forms such as HHABN and Medicare Notice of Non-Coverage accurately and timely
- Use McKesson workflows for visit locking, follow-up notes, and visit completion
- Coordinate safe discharges and transfers and communicate necessary documentation to the care team
- Participate in monthly peer review audits and engage in constructive feedback
- Schedule and facilitate case conferences to improve communication and outcomes
- Provide hospice coverage when scheduled on-call
Qualifications
- Active Registered Nurse (RN) license in Vermont or eligibility for Vermont licensure
- Experience with home health nursing and OASIS documentation is strongly preferred
- Comfort and competence with IV therapy, wound VACs, and complex wound care
- Knowledge of infection control practices and ability to apply them consistently
- Familiarity with McKesson or similar home health documentation systems
- Ability to serve as a case manager and manage productivity expectations
- Strong communication skills for patient education and interdisciplinary coordination
- Willingness to provide hospice coverage when on-call
Education
- Graduate of an accredited nursing program (ADN, BSN, or equivalent)
Knowledge & Skills
- Proficient in OASIS documentation and timely, accurate charting
- Skilled in home health nursing assessments and care planning
- Hands-on experience with IV use and maintenance
- Competence in wound VAC management and complex wound care
- Understanding of HHABN and Medicare Notice of Non-Coverage processes
- Familiarity with infection control standards and regulatory compliance
- Ability to use McKesson workflows for visit completion and follow-up documentation
- Strong patient/caregiver education and interdisciplinary coordination skills
