Roles & Responsibilities
- Assist licensed physical therapists in delivering resident-centered rehabilitative care
- Implement therapeutic exercise programs and mobility training to improve function
- Perform safe transfers and gait training, emphasizing fall prevention and independence
- Document treatment sessions thoroughly and communicate progress to the care team
- Collaborate with nursing and interdisciplinary staff to coordinate resident care plans
- Educate residents and families on exercises, home programs and safe mobility
- Support outcome measurement and participate in quality improvement efforts
- Maintain professional standards, infection control, and facility policies at all times
Qualifications
- Current or compact-state Physical Therapist Assistant license (NH licensure required or ability to obtain)
- Graduation from an accredited PTA program
- BLS/CPR certification (or willingness to obtain upon hire)
- Comfortable working in a skilled nursing facility setting; any experience level welcome, including new grads
- Strong communication skills and ability to collaborate with an interdisciplinary team
- Professional, reliable, and committed to resident-centered care
Education
- Associate degree from an accredited Physical Therapist Assistant program (required)
Knowledge & Skills
- Therapeutic exercise prescription and progression
- Gait training, transfers, and functional mobility techniques
- Clinical documentation and rehab outcome tracking
- Knowledge of SNF workflows and interdisciplinary collaboration
- Patient safety, fall prevention, and assistive device training
- Compassionate patient education and motivational coaching
