Roles & Responsibilities

• Assist licensed physical therapists in delivering resident-centered rehabilitative care
• Implement therapeutic exercise programs and mobility training to improve function
• Perform safe transfers and gait training, emphasizing fall prevention and independence
• Document treatment sessions thoroughly and communicate progress to the care team
• Collaborate with nursing and interdisciplinary staff to coordinate resident care plans
• Educate residents and families on exercises, home programs and safe mobility
• Support outcome measurement and participate in quality improvement efforts
• Maintain professional standards, infection control, and facility policies at all times

Qualifications

• Current or compact-state Physical Therapist Assistant license (NH licensure required or ability to obtain)
• Graduation from an accredited PTA program
• BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable working in a skilled nursing facility setting; any experience level welcome, including new grads
• Strong communication skills and ability to collaborate with an interdisciplinary team
• Professional, reliable, and committed to resident-centered care

Education

• Associate degree from an accredited Physical Therapist Assistant program (required)

Knowledge & Skills

• Therapeutic exercise prescription and progression
• Gait training, transfers, and functional mobility techniques
• Clinical documentation and rehab outcome tracking
• Knowledge of SNF workflows and interdisciplinary collaboration
• Patient safety, fall prevention, and assistive device training
• Compassionate patient education and motivational coaching
