Roles & Responsibilities

Assess patient needs in the home setting and create individualized care plans
Provide skilled nursing care including wound care, medication management, and symptom control
Deliver compassionate hospice support when appropriate, honoring patient goals
Coordinate care with therapists and interdisciplinary team members to ensure continuity
Educate patients and families to empower them in managing conditions at home
Document thoroughly and accurately in the patient record and follow agency policies
Advocate for patient needs and communicate changes promptly to the care team

Qualifications

Current RN license in North Carolina (required)
Home health or hospice nursing experience, serving Franklin/Macon County patients
Basic Life Support (BLS) certification required; hospice/home health certifications preferred
Comfortable working independently in patients' homes and managing caseloads
Valid driver’s license and reliable transportation for home visits

Education

Associate Degree in Nursing (ADN) required, BSN preferred

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Experience with home health documentation and care planning
Excellent communication and patient/family education skills
Ability to collaborate with therapists and interdisciplinary teams
Time management and organizational skills for field-based work
