Roles & Responsibilities

• Provide high-quality, compassionate in-home nursing care to adults and older adults
• Conduct thorough patient assessments and develop individualized care plans
• Deliver skilled nursing procedures and therapies safely and efficiently
• Coordinate care with therapists, physicians, and multidisciplinary team members
• Document clinical findings, treatments, and progress accurately in the EMR
• Educate patients and families on disease management, medications, and safety
• Manage caseload and prioritize visits to ensure timely, patient-centered care
• Maintain professional standards, infection control practices, and required certifications

Qualifications

• Active North Carolina RN license or appropriate therapy license (PT/OT/SLP) as applicable
• Home health experience required for therapy positions and strongly preferred for nursing
• Current BLS (Basic Life Support) certification
• Relevant advanced skills and certifications as applicable to the role
• Valid driver's license, reliable vehicle, and willingness to travel to patient homes
• Comfortable working independently and as part of a collaborative team
• C2C and W2 engagement options available

Education

• Associate Degree in Nursing (ADN) required for nurses; BSN preferred
• Degree in Physical Therapy, Occupational Therapy, or Speech-Language Pathology for therapy roles as applicable

Knowledge & Skills

• Strong clinical assessment and care planning skills specific to home health
• Excellent patient and family education and communication abilities
• Proficient documentation skills and familiarity with electronic medical records
• Good time management and route planning for efficient home visits
• Knowledge of infection control, medication administration, and wound care
• Ability to adapt interventions to the home environment and patient needs
